The answers of questions 3 and 4 in the Dictation section of the Exercises, are using different forms of "求“ and 哪 from that used in the expansion sentences and in the multiple choice questions。Are these incorrect or just another way to use the terms?

I enjoyed this lesson. John mentioned, at the beginning, that the structure of the rhetoric question was to use 难道 as a marker to start a "吗" question. In the examples there are three that do not end in "吗".
难道你没有时间?
难道你疯了?
难道你周末还要上班?
Is there a reason that these do not follow the pattern? Also, what is the purpose of the 了 after 疯?
Could that sentence also be said, 难道你疯吗?
